This Telehealth Informed Consent (“Consent”) describes your rights, responsibilities, and the risks associated with receiving healthcare services via telehealth through ZZMed LLC (“ZZMed”).
By proceeding with telehealth services, you acknowledge that you have read, understood, and agree to this Consent.

- Consultation, evaluation, and treatment recommendations

- Prescription of medications, if clinically appropriate

2. Role of ZZMed
ZZMed is a platform that facilitates access to licensed healthcare providers.
- ZZMed does not provide medical care, diagnosis, or treatment
- All medical services are provided by licensed providers or partner medical groups
- Any provider-patient relationship is between you and the provider, not ZZMed

4. Risks and Limitations
You understand and acknowledge the potential risks of telehealth, including:
- Limitations compared to in-person examinations

- Potential risks to privacy despite reasonable safeguards
Telehealth may not be appropriate for all medical conditions.
5. No Guarantee of Outcomes
You understand that:

- No guarantees are made regarding outcomes
6. Privacy and Security
Your information may be transmitted electronically as part of telehealth services.

- No system is completely secure
- There are inherent risks with electronic communication
Your information will be handled in accordance with applicable privacy laws and the ZZMed Privacy Policy.
7. Communications
You agree to receive communications related to your care, including:

8. Alternatives to Telehealth
You understand that alternatives to telehealth include:
- In-person medical visits
- Delaying treatment

9. Emergency Situations
Telehealth services are not appropriate for emergency conditions.
If you are experiencing a medical emergency:
- Call 911 immediately
- Seek in-person medical care
10. Right to Withdraw Consent
You may withdraw your consent to telehealth services at any time by discontinuing use of the Services.
However, withdrawal may limit your ability to receive care.
